MHF will provide a rebate of up to $250 per night, towards a 3-night stay at your choice of accommodation in Mildura and surrounding areas. Accommodation would need to be 30km of the Mildura Health Fund office, 79 Deakin Avenue, Mildura for one of our experienced midwives to visit you during your stay.

The in home program offers you the services and support you need in the comfort of your own home. This option is designed to allow you to spend more time getting to know your newborn and time to catch up on your rest.

To be eligible for the MHF Post-natal service, you must:

• Hold a MHF Five Star Gold Hospital product;

• Have served the 12 month Pregnancy and birth waiting period;

• Be financial and paid past your expected due date;

• Register your interest in this program at least 4 weeks prior to your expected due date;

• Receive the Home Stay at a private residence within a 30* kilometre radius of our Mildura Office at 79 Deakin Avenue Mildura;

• Have not taken part in another post-natal program or Medi-hotel stay.

• Give birth at Mildura Base Public Hospital and;

• Mother and Baby deemed fit for discharge by Mildura Base Pubilc Hospital.

Midwife services

• MHF Midwife services can only be provided after hospital and post domiciary care discharge from Mildura Base Public Hospital.

Following the birth of my baby, I consent to:

Post-natal visits and phone calls from the Mildura Health Fund (MHF) midwife or lactation consultant. Information, support and monitoring of my maternal recovery, including infant feeding, wellness and general parenting advice.

Acknowledgement 
If there are circumstances where my health and the health of my baby require assessment or treatment that cannot be provided by the Mildura Health Fund midwife or health care professional, I agree to be guided by them to seek assistance from the Mildura Base Public Hospital/Broken Hill Base Hospital/Swan Hill District Health. I understand this may mean returning to the hospital via the emergency department or directly to the post-natal ward. If directed by the Mildura Health Fund Midwife or health care professional or on any occasion where I am significantly concerned for my or my baby’s health, I will call an ambulance immediately. 

While participating in this program I consent to the midwife and health care professional reviewing and taking clinical notes to record my progress and care. I am aware these notes will be securely stored at the Mildura Health Fund office, in accordance with Mildura Health Fund Privacy Policy. A copy of my clinical notes may also be provided to, or discussed with, where medically necessary, my doctor, GP, lactation specialist or any other relevant medical professional. Mildura Base Public Hospital/Broken Hill Base Hospital/Swan Hill District Health may also receive a copy of my clinical notes whilst under the care of the Mildura Health Fund midwife or health care professional.

Declaration

I understand that; 

• I have the right to make decisions about my medical care;

• I may withdraw my consent at any time prior to treatment or care;

• I will receive advice to assist with my care and my baby’s care;

• I am responsible for my and my baby’s care;

I acknowledge that MHF have provided me with all contact details I may require during the program, including urgent or emergency contact numbers.

Acccomodation:

• I understand that by selecting the accomodation option that I am responsible for making my own accomodation arrangements. Mildura Health Fund will not make any reservations on my behalf.

• I agree to pay any accomodation charges to the accomodation provider I have chosen, and will submit a reimbursment after my stay. Mildura Health Fund will reimburse up to $250 per night (Max 3 nights) into my nominated bank account. I understand that any in-hotel charges are my responsibility.
  

• I understand that if I select Indugle Apartments or All Seasons Caravan Park, they will bill Mildura Health Fund directly for up to $250 per night (Max 3 nights), and any other in-hotel charges are my responsibility.
